1. General
At Narhex your privacy is important to us. We are committed to ensuring that information we hold about you is held securely and that your personal information is protected.
2. What information does Narhex collect about you?
The types of information we collect will depend on the types of products or services we provide to you. Narhex generally collects and holds personal information about:
(a) contractors who provide services to Narhex;
(b) our customers;
(c) our suppliers;
(d) job applicants; and
(e) other people who may come into contact with Narhex.
The types of personal information Narhex collects varies depending on the purpose for collection and your interaction with us, and may include:
(a) your name, address, phone/fax numbers and email address; and
(b) other information relating to the provision of Narhex services.
We only collect personal information that is needed by us to provide our products and services. We will usually collect your personal information directly from you, unless it is not possible for us to do so, or you have consented to us obtaining the information form a third party (eg a referee if you are a job applicant or from a referring doctor or health care provider).
3. Use and disclosure of your personal information
We may use your personal information for the following purposes:
(a) to provide products and services that you have requested;
(b) to maintain relationships with our suppliers and contractors;
(c) to facilitate our internal business operations, including fulfilment of any legal requirements; and
with your consent:
(d) to communicate special events;
(e) for market announcements;
(f) to conduct marketing research;
(g) for research and training; and
(h) for training and education.
Narhex will not disclose personal information about you to any person, except as required by law or on a confidential basis to agents that we use in the ordinary course of our business such as for data processing, printing or mailing.
You may at any time opt out of receiving our communications from us (other than as required for the operation of our business). Notification in writing of this option would be appreciated.
Narhex does not sell, rent or trade personal information about you to or with any third parties.
In the majority of cases, if you do not provide the information about yourself that BSES has requested, Narhex may not be able to provide you with the relevant product or service.
4. Information security
We take all reasonable steps to protect your personal information from misuse, loss, unauthorised access, modification or disclosure. This includes using electronic and physical security measures. For example, our electronic databases require log-ins and passwords.
5. Storage
Narhex stores personal information:
(a) contained in paper based and other hard copy documents located securely within the practice. All practices have security alarms which are in operation outside business hours;
(b) contained in electronic records in a secure environment; and
(c) archived to a dedicated storage facility.
Such records are only accessible by those persons who require access to the personal information for the purpose of carrying out their employment activities.
6. Access and correction
You may request access to personal information we hold about you by completing a request to access record form. If you wish to access the records please contact the Privacy Officer, who will arrange for the appropriate request for access form to be provided to you. Where we hold information that you are entitled to access, we will endeavour to provide you with suitable range of choices as to how you may access the record, such as allowing you to personally inspect the record, take notes or copy the record or you may request us to forward a copy of the record to you. We will endeavour to process any request for access within 30 days of having received your request. On collection, we will ask for some means of photo identification eg drivers licence, passport etc. We are entitled by law to charge you a reasonable fee for collating, copying and providing you with access to the records.
Narhex may refuse access to records in a number of circumstances. If we deny your request for access to personal information then we will give you our reasons for denial in writing.
If you believe that personal information we hold about you is incorrect, incomplete or inaccurate, then you may request amendment of it. We will consider if the information requires amendment. If we do not agree that there are grounds for amendment, then we will add a note to the personal information stating that you disagree with it.
Upon receiving your feedback, we will take all reasonable steps to update and correct the information promptly.
